Soriatane (acitretin)

Soriatane Description

Soriatane is used to treat severe forms of psoriasis, which is associated with the formation of bubbles or large areas of reddish inflammatory skin. Its main ingredient is acitretin. Acitretin, an organic chemical compound and a synthetic analogue of retinoic acid, is used in the symptomatic treatment of severe psoriasis. It is in a group of retinoids (vitamin A derivatives). It is involved in the control of growth and maturation of healthy epidermal cells and affects certain immune reactions in the second dermis. These two mechanisms lead to the formation of the epidermal cells. New epidermal overgrowth can be prevented by the containment of horny scales and existing epidermal overgrowth can be replaced within days.

Soriatane reaches its highest plasma concentration after one to four hours of its administration. Food, especially rich in fats, contributes positively to its absorption. The average bioavailability of the drug is, on an average, about 60%, but it is highly dependent on individual factors and can range from 35% to 90%. Since it is a very lipophilic substance, it penetrates well into tissues and crosses the blood-placenta.

Soriatane is metabolized to form the cis-isomer (13-cis-Soriatane), which then undergoes several metabolic reactions. The final stage of the metabolic pathway is the disconnection of the side chain. Small amounts of the drug undergo biotransformation to form etritinate. Although Soriatane is completely cleared from the body after 36 days, patients treated with this drug should be checked for the possibility of the formation of etritinate, whose half-life of approximately 120 days.

Conditions Treated by Soriatane

Further applications of Soriatane are employed in severe keratinization disorders, like the Darier's disease (hereditary disease characterized by skin and mucous membrane changes). It is also used to treat lichen planus, fish scale disease (ichthyosis) and other skin diseases with excessive keratinization, such as hyperkeratosis. It also helps in the formation of healthy skin cells and reduces skin flaking to a large extent.

Soriatane Dosage Information

a.) Typical Dosage Recommendations

The dosage should be adjusted according to the body weight of the person. Soriatane is not well suited to aged patients.

Soriatane can cause serious birth defects in an unborn child, and therefore its use during pregnancy is strictly prohibited. General Soriatane should not be prescribed for women of childbearing age. In case a Soriatane medication cannot be avoided for certain medical reasons, the woman must be informed by the doctor thoroughly about the risks. You must use appropriate means of contraception. Pregnancy must be consistently avoided until two years after completion of therapy with Soriatane. Acitretin is excreted in breast milk. Therefore, lactating mothers should also not take the drug.

b.) Missing A Dose

Rare side effects

Corneal ulcers, corneal inflammation, stomach problems, intestinal problems like vomiting, abdominal pain, diarrhea, indigestion, liver inflammation (hepatitis) with skin yellowing (jaundice), increased light sensitivity of the skin, intracranial pressure (pseudotumor cerebri) with headache, dizziness, nausea, dizziness, blurred vision are extremely rare side effects.

Possible Drug Interactions With Soriatane

If Soriatane and phenytoin (Dilantin) (one of the AEDs) are taken together, the effect of phenytoin is strengthened. Simultaneous consumption of alcohol causes increased conversion of Soriatane to etretinate. This is also part of etretinate in the group of retinoids, and shows more side effects than Soriatane.

If a methotrexate (Trexall) drug is taken along with Soriatane, the risk of liver inflammation is increased. Tetracyclines (antibiotics) should not generally be co-administered with Soriatane. This may be accompanied by impairment of consciousness, difficulty in breathing and slow pulse. The contraceptive effect of low-dose estrogen-progestin combinations (birth control pill) can be reduced by Soriatane. The temporary choice of another means of contraception is therefore warranted.
