Trusopt Drug Description

Trusopt is the commercial brand name of dorzolamide hydrochloride which is a basically an antiseptic ophthalmic solution used topically for the treatment of glaucoma of the open angled kind, ocular hypertension, and exfoliating glaucoma. Dorzolamide hydrochloride is the main and active ingredient of Trusopt and functions as a carbonic anhydride inhibitor. Dorzolamide hydrochloride is an off whitish (and sometimes whitish) crystalline powder that is highly soluble in water, and partially soluble in ethanol and methanol. Its molecular weight is 360.9 with an approximate melting point of 264 degrees centigrade. Trusopt ophthalmic solution is manufactured, marketed and supplied by Merck Sharp & Dohme Ltd. as a liquefied dorzolamide hydrochloride solution that is isotonic, germ-free, and somewhat viscous. Every ml of a 2% formulation of Trusopt has roughly 20 mg of dorzolamide. Besides dorzolamide, there are other components like mannitol, hydroxethyl cellulose, sodium citrate dihydrate, and sodium hydroxide that are all inert. For increasing the shelf life of the preparation, benzalkonium chloride is added.

Conditions treated by Trusopt

Trusopt is exclusively used for lowering the accentuated pressure in the eye that results from an ophthalmic condition called open angled glaucoma when there is a reduced secretion of aqueous humour in the ciliary body of the eye. A decreased secretion of aqueous humour subsequently leads to reduction in fluid transport and eventually the increased IOP (intraocular pressure) is normalized. A spurt in IOP is a significant risk factor that might lead to the pathogenic condition of open-angled glaucoma resulting in optic nerve damage and loss of vision. Trusopt deactivates an enzyme known as carbonic anhydrase which augments the production of aqueous humour. By bringing down the secretion of aqueous humour, the IOP is also kept under check. Truspot ophthalmic solution is generally used when a normal eye-drop beta-blocker is ineffective in lowering the eye pressure sufficiently.

Trusopt Dosage Information

a) Typical dosage recommendations

The usual dosage is just a single drop of Trusopt solution in either or both the affected eyes at least thrice a day. You can use this ophthalmic solution along with other ophthalmic formulations but there should be at least a ten minute gap between their administrations. Since the dorzolamide present in the Truspot solution is released into the bloodstream soon after administration, the compound might affect the functioning of other prescription medications that you might be taking. Therefore it goes without saying that you should check with your physician before you can start using Truspot. You’d also need to consult him before using other medicines along with Truspot to make sure that its efficacy is not negated. Wash and dry your hands thoroughly before you apply the drops. The Safety Strip of the bottle should be intact before you can use the solution. While administering the dosage, just push back your head sufficiently so that you can see the ceiling, press down lower eyelid and be cautious enough to pour just a drop. The tip of the dropper should not come in contact with any part of the eye under any circumstances. Replace cap firmly after use and store in a cool dry place.

b) Missing a dose

If you forget to take your usual dose irrespective of the reason, you should administer the same the moment you recall missing the dose. And in case, if it is too late by the time you remember or it is time for your next dosage, you can forego the missed dosage. Do not put extra drops to replenish the spell you missed.

c) Overdosing

Needless to say, in case of an overdose (where you’ve put more than one drop or have taken dosages more than the prescribed number of times), you’d need to go for emergency medical intervention. An overdose may result in an electrolytic imbalance and increased acidosis, and even the smooth functioning of the CNS (central nervous system) might be affected. The symptoms of overdose generally include dryness of mouth leading to your feeling thirsty, restlessness, vomiting, vertigo, increased micturition, muscular spasms, convulsions, fainting, and increased heart rate.

Trusopt Warnings

Trusopt containing dorzolamide hydrochloride belongs to the sulphonamides group of drugs. So, even if this ophthalmic formulation is administered topically (as contrasted to intravenous injection or swallowing), the solution ultimately enters your system and so the likelihood of suffering from adverse or undesirable reactions, that are typical of sulphonamides, remains. Although very rare, but fatalities have resulted from adverse reactions including but not limited to aplastic anaemia, epidermal necrolysis, fulminant or severe hepatic necrosis, and agranulocytosis. In case you use contact lenses, remove the same before you can use the solution. You can put on your lenses 15 minutes after administration. Pregnant and nursing mothers should exercise caution before using the drug and seek medical consultation.

Since the solution is sterile in nature, the risk factors associated with using the medication are markedly low, but nevertheless you should make yourself aware of its side effects. Following side effects are most likely to occur when the dorzolamide solution is imbibed in your bloodstream:

• Hives
• Swelling of eyelids
• Redness of eyes leading to pain, light sensitivity and discomfiture
• Oozing or drainage of the eyelids or eyes
• High fever, dryness of mouth or throat, burning sensation in your eyes, purplish or reddish skin rashes on the upper part of your body leading to blistering that are symptomatic of acute skin reaction
• Appetite loss
• Yellowing eye or skin that is symptomatic of jaundice
• Abdominal pain
• Dark colouration of urine
• Blurry vision
• Nausea
• Watery eyes
• Dryness in the eyes
• Headaches
• Listlessness and fatigue
