Atacand (candesartan)
Atacand Description
Atacand is the trade name of the drug Candesartan cilextil. It belongs to the group of sartans, which are Angiotensin II receptor antagonists. It is prescribed primarily for the treatment of hypertension and is administered alone or concomitantly with other drugs. In some cases, it is also used for the treatment of heart failure.
Conditions Treated by Atacand
Candestran is an Angiotension II receptor antagonist. An antagonist blocks the activation of the receptor, in this case the Angiotension II receptor. This causes a cascade, which results in reduced secretion of vasopressin, and aldosterone and causes vasodilation. The combined effect of all these is a decrease in the blood pressure. An angiotensin-converting enzyme (ACE) converts Angiotensin I into Angiotensin II. Angiotensin II acts on the renin-angiotensin system. This needs to be prevented in case of individuals suffering from Hypertension. For this, ACE inhibitors are used. However, ACE also degrades bradykinin. ACE inhibitors and leads also inhibit this reaction to complications. Candestran is thus a better option for controlling hypertension, as its mode of action is safer.
Atacand Dosage Information
Atacand is sold in the market in the form of tablets. These tablets are available in various doses: 4, 8, 16 and 32 mg of candestran.

The dosage should be personalized for each individual. The recommended starting dose is 16mg, once a day. It may also be given in varying doses ranging from 8mg to 32 mg depending on the patients requirements. The drug does not show any increased efficiency at higher doses beyond 32 mg. The effect of the drug is seen within 2 weeks.
For heart failure, the recommended starting dose is 4mg. This is to be doubled every two weeks until you achieve a dosage of 32mg.

Atacand Warnings
This medication is not to be consumed if you are allergic to any of the components of the medication as it may lead to complications. It is to be strictly avoided in case of pregnancy. The drug acts on the renin-angiotensin system. This causes fetal and neonatal morbidity when administered to pregnant women. Similarly, in infants who are younger than one year should not be administered the drug as it can affect the development of their kidneys. This can lead to complications further in their life.
Patients who are being treated with diuretics may be volume and/or salt depleted. Consequently, in such patients the renin-angiotensin system is active. Such patients, if administered the drug suffer from symptomatic hypotension. To prevent this, the condition must be corrected before administering the drug. If this is not possible, then the therapy with Atacand must be initiated with caution. In case, hypotension occurs, the patient must be treated accordingly.
In patients with moderate hepatic impairments, the recommended initiating dose is much lower as there is an increased retention of the drug in such patients. As the drug inhibits the renin-angiotensin system, it may affect the renal system in some individuals. It may cause problems such as oliguria or progressive azotemia. Progression of this condition may cause acute renal failure and may also cause fatalities in some cases. In patients, who are being treated with Atacand for heart failure, hyperkalemia might occur.
